Interested presenters must submit their application to present to the HAI Selection Committee by the published due date. The application includes a 1-2 page Executive Summary, completion of the proprietary HAI Company Screen Form, and your company's slide set. To obtain the Screen Form, email Mary Fox at: mary.fox@cstionline.org
